This form can be used to document any changes to contractual terms (without having to issue a new contract of employment). For example it can detail if a new notice period has been agreed, changes in hours of work or changes in holiday entitlement etc. The form allows you to detail relevant changes and allows for signatures by the employee and the Company. This is then attached to the original contract as an 'addendum'.
